比较18F-FDG PET/CT显像IHP、Deauville和△SUVmax在恶性淋巴瘤早中期治疗敏感性判定和预后评估中的应用价值

摘要: 目的 探讨IHP标准视觉判断法、Deauville 5分法和最大标准摄取值下降率(△SUVmax)法三种图像分析方法对恶性淋巴瘤患者的治疗早-中期敏感性判定和预后评估的价值。方法 选取符合入选标准的恶性淋巴瘤患者51例。分别采用IHP标准视觉判断法、Deauville 5分法对患者PET/CT图像进行分析。计算患者治疗前、治疗早中期△SUVmax,通过ROC曲线取得PET/CT对治疗早-中期敏感性判定和预后评估△SUVmax最佳临界值。对比这三种方法诊断效能之间的差异,并采用Kaplan-Meier生存分析、单因素和多因素Cox比例风险回归模型分析恶性淋巴瘤患者DFS的影响因素。结果 ROC曲线分析结果显示△SUVmax为56.95%时PET/CT诊断效能最佳。IHP标准视觉判断法、Deauville 5分法与△SUVmax法对淋巴瘤治疗早-中期疗效评估均可获得较高的灵敏度和阴性预测值,其中△SUVmax法的阳性预测值和准确度均高于另外两种方法。结论 IHP标准视觉判断法、Deauville 5分法和△SUVmax在淋巴瘤治疗早-中期的疗效评估和预测预后方面均有较高的临床价值,均可作为判断预后的独立危险因素。

Abstract: Objective The aim of this study was to explore the values of IHP standard visual judgment,deauville(5-porint scoring and 5-PS)and maximum standard uptake rate(△SUVmax)image methods in the evaluation of early-and mid-term sensitivity determination and prognosis in malignant lymphoma patients. Methods A total of 51 patients with lymphoma who met the inclusion criteria were selected for retrospectively analyses.PET/CT images of patients were analyzed by IHP standard visual judgment and deauville five-point methods.The △SUVmax value of the patient in the early and middle stages of treatment was calculated before treatment, and the optimal cut-off value of PET/CT for early- and mid-term sensitivity determination and prognosis of evaluation △SUVmax was obtained by ROC curve.The differences between the diagnostic efficiencies of three methods were compared.The Kaplan-Meier analysis and Cox proportional hazard regression model were used for univariate and multivariate analyses. Results The results of ROC curve analysis showed that PET/CT had the best diagnostic efficiency when △SUVmax was 56.95%.IHP standard visual judgment,deauville quintile and △SUVmax methods could obtain high sensitivity and negative predictive values for the early-term treatment of lymphoma treatment.The positive predictive values and accuracy of △SUVmax method were higher than other two methods. Conclusion IHP standard visual judgment,deauville 5,and △SUVmax have high clinical values in the evaluation of early and middle stage of lymphoma treatment and prognosis.They can be used as independent risk factors for prognosis.
