Clinical pathologic analysis on 678 cases of salivary gland tumors

Objective To study the characteristics and tendency of incidence of salivary gland tumors during the past 28 years and to provide information for clinicians and pathologists.Methods The data of tumors in salivary gland were collected from General Hospital and the Second Hospital of Tianjin Medical University.The data were stored in Excel and were analyzed by Spss11.5.Results The incidence of salivary gland tumors was increasing during the past 28 years.The relative ratio of male to female was 1.030∶1.The ratio of the benign cases to the malignant ones was 4.795∶1.Benign tumors are more in age of 40-, 50-, and malignant tumors are more in age of 50-, 60-.The parotid gland(70.94%)was the most common location of salivary gland tumors.Pleomorphic adenoma(69.34%)and adenoid cystic carcinomas(35.90%)were the most frequent benign and malignant tumors, respectively.Conclusion The incidence of salivary gland benign and malignant tumors were increasing during the past 28 years.The parotid gland was the most common location both of benign and malignant salivary gland tumors.Pleomorphic adenoma was the most common histological type in the salivary gland.
Radiation therapy in the management of patients on Epidemic Kaposi′s Sarcoma associated with AIDS

Objective To analyze retrospectively radiotherapy efficacy in Epidemic Kaposi Sarcoma(EKS)in patients associated with acquired immunodeficiency syndromes(AIDS).Methods Total 659 patients with AIDS-related EKS were treated using radiation therapy at the Department of Radiation Oncology of Mondor University Hospital between July 1987 and June 1997, the median follow-up period was 3.2 years.Of which, 398 patients(60.3%)had previously received treatment, either single or combined with interferon, vinblastine, doxorubicin, bleomycin, and/or antiviral treatment.The irradiation was delivered by an accelerator of 4 MeV-X ray or 8 MeV electron beam for extended cutaneous fields, or by 45-100 kV x-ray for very localized fields(oral cavity).The dose delivered was usually 20 Gy in 2 weeks(2.5 Gy/fraction, 4 fractions/week)followed by 2 weeks rest and then second serie of 10 Gy added within 1 week.For oral cavity lesions, a series of dose 15.2Gy was delivered(1.9 Gy/fraction, 4 F per week), of these, 3 patients followed by a 2-3 week rest and then received a similar second serie of 15.2 Gy.Results 629 patients were statistically evaluable and the Objective response rate was 92%, with a complete regression of clinical symptoms for all irradiated patients.The skin and mucosa tolerance was good.There was an obvious correlation between tumor recurrence rate、survival of the patients and the occurrence of opportunistic infections.Conclusion Radiation therapy is an efficient method in the management of epidemic Kaposi sarcoma with AIDS, and good quality of life and improvement of symptoms were achieved after radiotherapy.Meanwhile, proper care and restriction on irradiation dosage and technique used should be advocated in advanced EKS patients associated with AIDS.
Feasibility of splenectomy in cytoreduction for epithelial ovarian carcinoma

Objective To investigate feasibility and necessity of splenectomy with tumor cytoreduction for advanced or recurrent epithelial ovarian carcinoma.Methods Seven cases undergoing splenectomy as part of primary cytoreduction(5 cases)or secondary cytoreduction(7 cases)were analyzed retrospectively.Results Of the 12 cases, mean survival was 37.75 months(13-65months)from the initial diagnosis, 3-year overall survival rate was 58.33%(7/12);3-year disease free survival rate was 16.67%(2/12).Post operation complication rate was 25%(3/12), and the preoperative complications related to splenectomy was 8.33%(1/12), the mean survival from initial diagnosis in optimal cytoreduction(7 cases)and suboptimal Cytoreduction(5 cases)was 43.33 months and 21 months(t=3.215, P<0.05)respectively.There was significant difference in survival between these two groups.Conclusion In order to get optimal cytoreduction for advanced and recurrent ovarian carcinoma, it is necessary and feasible to perform splenectomy for spleen metastasis.
Clinical significance and change of IL-13、TGF-β2、INF-γ and IL-21 in aGVHD

Object To study the effect of cytokine INF-γ, TGF-β2, IL-13 and IL-21 on acute graft-versus-host disease(aGVHD)after allogeneic hemopoietic stem cell transplantation(allo-HSCT).Methods The aGVHD was observed in 20 patients with allo-HSCT.The peripheral blood of 20 patients was collected before and after transplantion, and the concentration of INF-γ, TGF-β2, IL-13 and IL-21 was detected by enzyme-linked immunosorbentassay(ELISA).Result The haematogenesis of 20 patients recovered after the allogeneic hemopoietic stem cell transplantation.The median-time that neutrophilic granulocyte recovered to 0.5×109/L was 13.5 days, and the median-time that platelet recovered to 20×109/L was 18 days.aGVHD was found in 7 of 20 patients, including 4 patients withⅠ-Ⅱgrade aGVHD and 3 patients with Ⅲ-Ⅳ grades.In patients with aGVHD, the concentration of INF-γ and IL-21 after transplantation increased significantly than that before transplantation, while that of TGF-β2 decreased significantly.And the concentration of INF-γ and IL-21 in patients with aGVHD increased significantly than that in patients without aGVHD(P<0.01), however that of TGF-β2 decreased significantly(P<0.01).The concentration of IL-13 had no change.Conclusions INF-γ、IL-21 have the up-regulated function in aGVHD, and TGF-β2 has down-regulated function.It′s valuable to detect the concentration of INF-γ and IL-21 after transplantation for predicting aGVHD.IL-13 has no relationship with aGVHD.

Objective To study the cytotoxicity and inducing apoptosis effects of dendritic cells(DC)co-cultured with cytokine induced killer(CIK)cells against colon cancer cells lines LOVO.Methods Mononuclear cells were extracted from health adult peripheral blood.The cells were used to induce DC and CIK respectively with different cytokines, and after DC maturing these two kinds of cells were co-cultured.The proliferation activity of CIK cells were observed by cell counting method, the cytotoxicity was detected by MTT assay, transmission electron microscopy was used to observe the apoptosis of tumor cells, and the expression of FasL in effective cells was determined by Western Blotting method.Results DCs could advance the proliferation activity of CIK cells, and the killing activity of DC-CIK cells was significant higher than those of CIK cells alone at different effector-target ratios.Transmission electron microscopy showed that DC-CIK could induce colon cells to apoptosis, and quantity expression of FasL on effective cells could be observed.Conclusion DC cells could obviously enhance the proliferation and cytotoxicity of CIK cells, one of the mechanisms of cytotoxic activity in DC-CIK cells is displayed by Fas/FasL pathway.

Objective To analyze the characteristics of CT diagnosis in early pancreatic adenocarcinoma.Methods 34 pathologically confirmed cases of pancreatic cancer adenocarcinoma(22 Male cases, 12 Female cases, 63.7 years old on average)were all examined by CT diagnosis for the lesions′ size, shape, density and strength separately.Meanwhile, assessment of the situations, such as the expansion of pancreatic duct and bile duct, pancreas atrophy, metastasis and so on were carried out.The outcome of patients with pancreatic head lesion in 22 cases, 8 cases of pancreatic body, 4 cases of pancreatic tail.Lesion size ranges 15-60 mm, with an average of 35mm.23 cases of lesions strengthen unevenly.The outcome of plain scan with 5 isogram cases and 29 low density cases;11 low density cases and 4 isogram cases on arterial phase;8 low density cases and 7 isogram cases on portal phase.15 cases indicated bile duct expansion, and 23 cases indicated expansion of pancreatic duct.17 cases occured liver and peritoneal metastasis.Conclusion There is a certain degree of difficulty for CT scan diagnosis of small pancreatic adenocarcinoma on early stage.Optimization of scanning technology is the key to raise the level of diagnosis.

Objective To investigste the correlation between the positive rate of HPV genetypes and the severity of cervical lesions.Methods Flow-through Hybridization was employed to detect the positive rate of high-risk HPV and the genetypes of HPV in cervical lesions.Results The positive rate of HPV test in cervicitis, CINI, CINII, CINIII and cervical cancer were 43.87%, 64.70%, 91.67%, 93.75% and 96% respectively.The most common genetypes in cervical lesions were HPV 16, 18, 52, 58, 33 and 31.The positive rate of multiplicitas genetypes infection in cervicitis, CINI, CINII, CINIII and cervical cancer were 35.8%, 45.9%, 54.9%, 60.8% and 70.2% respectively.Conclusions The positive rate increased with the process developing of cervical lesions.The most common genetypes in cervical lesions were HPV16, 18, 52, 58, 33 and 31.There were more multiplicitas genetypes with the development of cervical lesions.

Objective To explore the value of FNAC in diagnosis of hyperplasia of mammary glands, fibro-adenoma and adenocarcinoma.Methods Pathological analysis were carried on the 208 cases with hyperplasia of mammary gland, fibro-adenoma and adenocarcinoma.Results The result manifests the diagnostic accuracy of the group is about 82%, which is basically the same as the data in the literature.Conclusions FNAC has a higher accuracy in diagnosis of the disease with hyperplasia of mammary gland, fibro-adenoma and adenocarcinoma and it is worth promoting.

Objective To assess the diagnostic value of three dimensional multi-slice spiral computed tomography angiography(3D-MSCTA)in detecting intracranial aneurysms.Methods The imaging data of 31 patients with intracranial aneurysms were retrospectively analyzed, 13 patients were performed with DSA, 4 patients were conformed by operation.Reconstruction types of MSCTA included volume rendering(VR), maximum intensity projection(MIP), multi-plane reformation(MPR)and curved plane reconstruction(CPR).Results In the 31 cases, 38 aneurysms were detected by MSCTA.Three cases had two aneurysms, two cases had three aneurysms.The locations of the aneurysm were middle cerebral artery(n=9), anterior cerebral artery(n=7), posterior cerebral artery(n=2), posterior communicating artery(n=3), carotid(n=12), basilar artery(n=3), corpus peripheral artery(n=1), superior cerebellar artery(n=1).The minimum diameter was 1.0mm×1.3mm, The maximum diameter was 59mm×73mm.MSCTA can precisely display the size, configuration and direction of intracranial aneurysms, and their anatomic connection with surrounding vessels and bones using 3D post-processing methods.Compared with DSA, 16 aneurysms were detected by MSCTA and 13 by DSA.One aneurysms was detected by DSA and was normal in MSCTA.The sensitivity, specificity, positive predictive value and negative predictive value for detection of aneurysms with MSCTA were 92.9%, 97.1%, 81.3%and99% respectively.Conclusion 3D-MSCTA has important significance in the diagnosis and operation for intracranial aneurysms.

Objective To observe the application of Tibolone in patients of cervical squamous cell cancer.Methods The levels of six reproductive hormone were evaluated by RI.The levels of TC and TG were determined by automatic biochemistry analyzer.The bone mineral density of lumbar vertebra was measured by QCT.The QOL was evaluated by 90 self-rating symptom table.Results Compared with placebo, the levels of E2 and BMD were significant higher in the group of Tibolone, but the levels of FSH, LH, TC and TG were low.Conclusion Tibolone improves climacteric syndrome efficiently in post-operative patients of cervical squamous cell cancer and the QOL.

Objective To investigate the effect of glutamine on the healing of colon anastomoses after early postoperative intraperitoneal chemotherapy.Methods Sixty rats were randomized into 3 groups:control group:only colon resection and sutured anastomosis was performed;early postoperative intraperitoneal chemotherapy(EPIC)group:5-Fu(20mg/kg·d)was injected intraperitoneally for 3 days after the operation;glutamine group:the EPIC was given and glutamine(1g/kg·d)was given from mouth for 7 days.All the rats were killed on the 8th postoperative day to investigate the adhesion formation score, the bursting pressure, and the macroscopical examination.Results The adhesion formation score of the early postoperative intraperitoneal chemotherapy group was significantly higer than that of other groups;the bursting pressure of the glutamine group was significantly higer than that of other groups;There was more fibroblast activity in the glutamine group than that in other group(P<0.05).Conclusion Using glutamine can improve the healing of colon anastomoses in the condition of the early postoperative intraperitoneal chemotherapy
